Last Minute Update
1. 譚建師母九月份診斷有腦瘤後, 手術切除了95%,還有5%靠著動脈,無法進行手術. 師母的身體還在恢復之中, 常常有头痛, 不宜多動. 她很抱歉地告訴中西部的弟兄姐妹, 這次她不能來和大家一起分享. 只能是張牧師一個人過來, 請弟兄姐妹繼續為她禱告. 我們也祈求神在將來的聚會中帶領師母來到我們中間. 這次大會的同工們決定取消教養子女的專題, 懇請大家的理解.
2. 這次大會場地附近也有很好的度假場所, 來參會的弟兄姐妹和朋友們可以在會前或會後同家人朋友一起過一個愉快的假日時光. 以下是這些場所的網頁連接:

3. All standard room has a coffee maker and a small fridge (all paid stuff should have already removed).
4. The hotel guests can have food in their own room but not in any of the meeting rooms or public area.

Registration Form Related Issues

1. 年齡計算以 12/26/2008 00:00AM 為基準。
   Age cut-off date is 12/26/2008, 12:00AM (mid-night).
   
   
2. 報名時全家填一張報名表即可，以六人為限 (含小孩) 。六人以上請加填另一張報名表。
   Please use one registration form for whole family; if more than six people, please use additional form.
   
   
3. 18歲以下青少年及兒童無父母或監護人陪同者，須有父母或監護人簽名之緊急醫療授權書方可報名。若有需要，請至下列網址下載緊急醫療授權書.
   Minors (under 18 years of age) who are not accompanied by the parents / guardians, are required to complete a registration form AND a Medical Release form with parent / guardians’ signature. Church Rep must check whether forms been completed. See Church Rep for forms or download from MCCA web site
   
   
4. 牧師、全職傳道人及他們配偶的所有費用全免(不含子女、父母) 。請在報名表[事奉]一欄中註明，以便正確計算實際所需費用。
   MCCA Pastors or full-time ministers and their spouses are free of charge (children or parents are not included).
   
   
5. 12/14/2008前，請各負責聯絡同工將貴教會/查經班所有報名表、附件及支票，以 certified mail 寄給 Paul Su, 7973 Hedgewood Cir. Mason, OH 45040 收，以便註冊組作最後資料處理。

取消注冊須知
Registration Cancellation Policy

1. 全額退款須於十二月十五日前提出，但註冊費恕不退還。
   Registration cancellations received prior to 12/15/2008 may be eligible to receive a full refund. But registration fee is not refundable.
   
   
2. 十二月十五日之後，恕不退款。
   Cancellations received after 12/15/2008 will not be eligible for a refund.
   
   
3. 所有取消事項，請由電郵或電話或傳真通知MCCA註冊組。
   Cancellations will be accepted via phone, fax or e-mail, and must be received by the stated cancellation deadline and by MCCA registration.
